A student organization must be registered with the Office of the Executive Vice President and Chief Academic Officer (EVPCAO) at The University of Texas Health Science Center at Houston (UTHealth Houston). To avoid renewal delays, please review the student organization manual, guidelines, procedures, and relevant HOOP policies located here, prior to completing this application.

Student organizations at UTHealth Houston are eligible for registration  under HOOP Policy 110, Student Organizations so long as its membership is only limited to enrolled students and employed staff and faculty at UTHealth Houston it does not deny membership on the basis of race (including hair texture or protective hairstyle), color, religion, sex (including pregnancy, childbirth, or related medical conditions), gender, sexual orientation, national origin, age, disability, genetic information, gender identity or expression, veteran status or any other basis prohibited by law or University policy. Except: an organization created primarily for religious purposes may restrict the right to vote or hold office to persons who subscribe to the organization's statement of faith; and it is not subject to any disciplinary penalty prohibiting registration; and it conducts its affairs in accordance with UTHealth Houston HOOP policies, and School rules.
 
Please complete this application form and an attached electronic copy of the organization’s bylaws (must be in MS WORD format, and follow the Sample Bylaws Template) will be sent to the Student Affairs Office of your school. The Student Affairs office will review the application prior to submission to the Dean for recommendation. Once approved by the Dean, Student Affairs will submit the application to the Office of the EVPCAO for review and institutional registration.

The Student Affairs office at your school will be notified by the Office of the EVPCAO regarding the registration renewal.

ATTESTATION: On behalf of the members of this proposed student organization, I certify that I have read and understand the policies of UTHealth Houston regarding registration of student organizations and that the information provided above is true and correct. The organization agrees to comply with said policies. As a condition of being a registered student organization, it must submit a completed renewal application at the beginning of each fiscal year (due by 11/1 of each year). Failure to comply with the policies will result in termination of the organization’s recognition as a registered student organization.
Certification of Members: I certify that this organization does not and will not during the 12 months following registration and approval have as a member any person who is not an enrolled student, employed faculty member, or employed staff member of UTHealth Houston. I understand that if I, or a representative of my organization, fails or refuses to sign this statement, or if the Dean determines that the statement is false, the Dean, after providing notice, shall begin disciplinary proceedings, and the organization’s recognition as a student organization will be withdrawn.
