INFORMED CONSENT FORM TO TAKE PART IN RESEARCH
Title: Genetics in the NICU: Nurses’ Perceived and Desired Knowledge

You are invited to take part in a research study called “Genetics in the NICU: Nurses’ Perceived and Desired Knowledge,” conducted by Kathleen Shields, of the University of Texas Health Science Center at Houston (UTHealth). For this project, Kathleen Shields will be called the Principal Investigator or PI.
The purpose of this research project is to describe the perceived medical genetics knowledge of nurses working in neonatal intensive care units, and to determine what further genetics education NICU nurses desire to support their work with patients undergoing genetics evaluations. If you decide to take part in this study, the total time commitment is 15-30 minutes. You are invited to take part in this study because you are a nurse working in a level 3 or 4 NICU.
The survey includes questions on demographic information, questions assessing your perceived knowledge of genetics concepts, questions assessing your comfort working in clinical scenarios involving genetics, and questions assessing desired genetics education.
Participation in this study is voluntary. You can refuse to answer any questions asked or written on any forms. Answers will remain anonymous.
You may not receive any benefit from taking part in this study. There are no known risks to take part in this study. The information collected will not contain identifying information. You will be offered to take part in a drawing, which will require your e-mail address in a separate link. If you agree to provide your e-mail address, the only risk may be breach of confidentiality. You have the alternative to choose to not take part in this study and can withdraw at any time.
There is no cost and you will not be paid to take part in this study. Upon completing the survey, you will be offered the option to enter your email address to participate in a raffle for one of four $25 Target gift cards. This information is not required. You will not be personally identified in any reports or publications that may result from this study.
