Please complete the information listed below to request an appointment to have the IDM hold removed. Once the form is submitted a staff member in the Student Affairs Office will reach out to schedule a ten-minute appointment during the window you requested. Please be sure that you are available during the time block requested and check your email regularly for appointment information. Appointment confirmations will be sent no later than three business days after an appointment has been requested. You must log into the session with a device that has a camera enabled and have an approved government ID available to present during the meeting.
Please do not schedule multiple appointments as this reduces the number of available appointments for all students. If you missed, your appointment window, please email the person who originally scheduled the session to determine a new date/time.
