To whom it may concern:

 

We are conducting a research project entitled,” Color matching, communication and reproduction survey”.

The purpose of this online survey is to compile information regarding color matching, communication and reproduction in the dental office and laboratory including the frequency and cost of shade mismatches.

As a part of the study, you will be asked to complete a survey that will take approximately 5 minutes.

 

If you agree to participate, you will click on the link that will take you to the survey.

There is no risk to you associated with this project and no personal information about you will be gathered during this study.  Your participation is strictly on a volunteer basis. There is no cost to take part in this study. You will not be paid for taking part in this study.
